It is important that both the nurses at Pacific Heights Surgery Center and your Surgeon have a complete history and physical prior to the day of surgery. We offer the convenience and privacy for our patients to complete their health history online. This will supplement the history you provide your Physicians. If you have surgery scheduled at our center, please fill out your information by clicking the provided link below. You will be asked about your health history, medications, and previous surgeries. It is important to complete this form as soon as possible so that our medical team can review your information prior to your visit.
